Order Form for New School Spirit Shirt

Dear Parents and Guardians,

We are thrilled to announce that the Home and School Association is providing a FREE school spirit shirt for every single student this year!

Whether it's for field trips, spirit days, or showing school pride in the classroom, we want every child to have one.

However, shirts are NOT ordered automatically.

Even though the shirts are completely free, parents must fill out the order form so we know exactly what size to get for your child. If you have multiple children at our school, please submit a separate entry for each student.

You may purchase an additional shirt if you would like, for $10 each (add $3 for 2XL, 3XL, 4XL).  You will still need to fill out the order form above and choose one of the payment options.

 Deadline to Order: 10/06/26

Please note: We cannot guarantee a shirt for your child or correct sizing if the form is not completed by the deadline. Let’s get 100% participation so no student misses out!
